U.S. Department of Labor Cites Alabama Plastics Manufacturer Following Employee Fatality

What was the cause of this accident?

-Employee was pulled into a plastics recycling machine

How could we have prevented this accident?

-A full occupational health and safety management system would have performed job safety analyses for all machines in the facility.  This would have identified the need for machine guarding for this specific machine.

Will my organization be fined?

In this scenario, this organization received a citation for failing to provide machine guarding.  In the process of evaluating the facility, additional fines were added for “failing to evaluate powered industrial trucks every three years, not having specific safety procedures to shut down or isolate stored energy; and failing to install a rail system on both sides of an open platform.”  If OSHA makes a visit to your facility following a fatality, they are likely going to look at every aspect of your organization to determine if you are in compliance.
